Getting closer to the completion of the DNP curriculum!





Two years of learning, the joy of meeting and collaborating with colleagues from across the globe and many contributions from CU Nursing Scholars, it's almost time to wrap up!


The Project

The culmination of every terminal degree ends in a scholarly project. The road to determining the project is not for the faint of heart. Without the cheerleading squad of colleagues, friends, family and educators undoubtedly no one would make it this far. At least that is what my experience has been. But like all things, we arrive. And arrive I have. My project is underway and the results are incredible! I am witnessing how small successive changes can yield tremendous outcomes for patients.


Coordinating Care for Patients With Chronic Care Needs

Inspiring

Coordinating care for patients with chronic health conditions does not seem like it should be complex. However, when those same patients touch many providers, across many systems, their care can quickly become complex. Patients are at risk for care fragmentation. Navigating ones health during a time of routine wellness is hard, managing health during times of illness is beyond challenging. For a nurse, it is inspiring to see how outcomes can be changed from something as small as handing of communication or building a plan of care. These individuals are inspiring. Serving them through compassion and coordination is truly a humbling and awe inspiring action. What an honor to be on this journey with my colleagues and so many patient champions!
